There are some tough malwares that neither anti-virus or spyware can't get read of. Removing startup entry may not help. Sometimes it only can be done manually. You probably need to identify processes spawned and what is spawning them. You would need a bit of experience with Windows. Watch your process list in task manager. Try to kill processes that is not part of windows process and see if that process spawns back. If they do, search the file for that name of the process. BEFORE deleting. search all files that has been created within same day in your harddrive and keep the information. Now you need to see you can identify process which spawns the process you killed. It maybe one of other files that was created about same time as other one. When ready, try to kill both processes and delete the files, check if same process comes back again. Some of the malware has spawn process that spawns spawn processes. On nasty case, identifying 2-3 file and deleting them won't help. Not only the process but the file will be regenerated mostly after reboot. Thus identifyng all files that has been created close time as target process/file is important. You will need to remove one by one until it no longer spawns back. Be catious not to delete legit files or kill legit processes.
